• Name(s) of Proprietor(s) Owner(s) Partner(s) and Office Address, Telephone / Fax Numbers.

• Relevant experience of Educational / Scientific documentaries (CDs of selected work MUST be enclosed).

• List of clientele.

• GST & NTN Registration Numbers
